Once a Music Medal assessment has been completed the Teacher-Assessor should fill in a report form providing feedback on the performance, an indication of the attainment categories awarded for each section and an overall result. The form will then be submitted to the Associated Board for moderation. The form is accessible from the Entries and Results website.
In order to pass the Medal, candidates have to achieve a Pass in a minimum of two of the three sections, of which one must be in the Ensemble section. From Bronze level, to pass ‘with excellence', all three sections must be awarded an Excellent result.

The report form should normally be completed during or just after the candidate’s performance, but this may not always be possible. If you have contributed to one of the performances, try to complete the form as soon after the Medal as possible, while it is still fresh in your mind. You may occasionally be unsure of the precise details of a performance (particularly, for example, if you were contributing to a quartet). In such cases you may need to make an initial draft assessment, which can be confirmed or adjusted later, following a viewing of the video recording once the Medal has been completed.
At the top of the report form, the personal details of the candidate are preprinted, to the right of which is the area where you indicate the overall result you have awarded.
Below this, the form is divided into three main section-headings for detailing which Ensemble and Solo pieces were played and which Option test was taken, along with the attainment categories you have awarded holistically for each section.
Under each heading there is a set of grids representing various aspects of the performance. There are four grids for the Ensemble, three for the Solo and one for each Option test. To make the process of reporting as straightforward as possible, you cross boxes in each grid to indicate particularly significant aspects of the candidate’s performance that have influenced your assessment within each section.
The crossing of the boxes does not relate directly to the attainment category awarded for each section but is solely for the purpose of constructing the commentary that each Music Medals candidate receives. This commentary is not written by you on the day but is produced after you return the report form to the Associated Board for moderation and is generated automatically from the boxes you crossed. For example, your crosses might produce the comment ‘You recovered promptly from slips and conveyed the intention of the piece' or ‘The notes were mostly correct, but there was also some uncertain intonation’.
The commentary is not diagnostic, but records some of your impressions on the day as Teacher-Assessor. By contrast, as teacher it is within the context of the group lesson that you provide any guidance to your pupils on practical ways in which they can improve.
There is no correlation between the number or distribution of crosses and the attainment category arrived at holistically for each section by the Teacher-Assessor.

If you wish to amend an assessment, you will need to print off and complete a fresh report form.
Report forms should always be kept out of pupils’ sight. Candidates should not be told the results you have awarded as they are subject to the Associated Board’s moderation process.
After you have finished the assessments, you should send the completed report forms and supporting paperwork, together with the video-recordings, to the Associated Board, following the instructions provided.
